Tuesday, Sept 7, 2022
Greetings,
Happy fall from the Pediatric Care Coordination: Community of Practice! Whether you’re supporting kids’ back to school transitions or just trying to remember where you stored your sweaters, we wish you well navigating this change of seasons.
In this newsletter, you’ll find:
A reminder to register for “Helping Families Thrive with Minnesota Help Me Connect,” our September Community of Practice webinar
Welcome to our new Advisory Group members!
Funding opportunity for a quality improvement project to improve birth outcomes
A sneak peek at our next webinar with PrairieCare’s Psychiatric Assistance Line (PAL)
We also invite you to regularly visit our website, http://www.mnpedcares.com to learn about relevant resources, upcoming events, and other information that will help you support kids and their families. Do you know about our members-only directory page? This directory lists Community of Practice members, searchable by location and job title. For example, you can search for members who work in your neck of the woods or who provide specific types of support (e.g., speech/language).
Look at all our members across Minnesota in the screenshot of the directory below! When you become a member, we invite you to choose to be listed in the directory for our members to connect more easily with each other.
As always, thanks for your participation in the Community of Practice. You make our work possible!
All our best,
Wendy Berghorst and the ACET team

Get funded for a QI project to improve birth outcomes: Applications due September 30
During well-child checks, access proven tools to screen, coach, and refer moms to services to ensure they have what they need to improve their health. Discover an innovative way to reach moms and screen them for tobacco use, depression, family planning, and folic acid intake. Your clinic can do this through the Minnesota IMPLICIT (Interventions to Minimize Preterm and Low Birth-Weight Infants Using Continuous Quality-Improvement Techniques) Program. Apply and become part of a statewide effort, which is part of a national endeavor of the Family Medicine Education Consortium, Inc.
Apply using the Minnesota IMPLICIT Program application form. Applications are due by 5 p.m. CST on Friday, September 30, 2022.
Chosen clinics will receive:
● Free technical assistance to implement the IMPLICIT interconception care toolkit;
● Free IMPLICIT trainings;
● Free support in providing continuous quality improvement to improve women’s health and prevent birth defects;
● Connections to other clinics in the program; and
● A $12,500 award over the course of the project.
Register for an informational webinar to learn more(12 p.m. on September 8). The Minnesota IMPLICIT Program is facilitated by ACET, Inc. under a grant funded by the Birth Defects Program at the Minnesota Department of Health. Learn more about this QI project on ACET’s website.
